Liability Waiver Contract

I, ______(NAME OF THE RELEASER)__, being of lawful age and sound mind in consideration of being permitted to participate in either online or in person Earth's Amulet sessions, classes, ceremonies, rituals, and related activities and programs run and/or operated by the Released Parties and/or any organized activities and programming on site or in the region and associated properties of said Released Parties do WAIVE, RELEASE and DISCHARGE the Released Parties, Earth's Amulet owners, officers, directors, employees, representatives, associates, members, agents, facilitators, and legal representatives of the above noted activity and each of them their owners, officers and employees, from all liability for or by reason of any damage, loss, or injury to person and property, even injury resulting in the death of the Releaser, which has been or may be sustained in consequence of the Releaser’s participation in the activities described above or sustained on the grounds and property of the Released Parties or during organized programming in regions conducted by the Released Parties, and notwithstanding that such damage, loss or injury may have been caused solely or partly by the negligence of the Released Parties.

I also acknowledge that by signing this release, I will be forever prevented from suing or otherwise claiming against the Released Parties for any property loss or personal injury that I may sustain while participating in or preparing for the above noted activity.

I understand that this Waiver and Release agreement is binding for me, my spouse, my heirs, my executors, administrators, personal representatives and assigns.

I acknowledge that I do not have any physical limitations, medical ailments, physical or mental disabilities that would limit or prevent me from participating in the above mentioned activity. If I have any such physical or mental disabilities that I am aware of that I will inform the Released Parties in writing before participating in such activities.

This release contains the entire agreement between the parties to this release and the terms of this release are contractual and not a mere recital.

Beverly Elizabeth Wheeler, and her facilitators intend to facilitate the sessions, ritual and/or ceremonies, rain or shine. However, I agree that Earth's Amulet are not liable for acts of weather, strikes, protests, or actions taken by government agencies and similar causes, which may interfere with program ceremonies or the classes taking place. I understand that my payment is nonrefundable and that this policy will not change even if the event is terminated early or canceled, or activity conditions are modified, due to harsh weather, acts of nature, governmental regulation, or conditions beyond control.

I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, Earth's Amulet and/or their directors, owners, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers.

(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.

(C) I acknowledge and agree as follows:

I confirm that the types of healing shared, discussed, and taught in this program or any other type of energy healing practiced by Earth's Amulet, whether in person or by proxy, is not intended to be a substitute for medical care. Additionally, these teachings and practices are not intended as medical advice, diagnosis, or treatment.

I confirm that I possess sufficient physical health and coordination to enable safe participation in the program and its ceremonies and activities.

I confirm that these rules, liability waiver, and indemnification provisions supersede any and all previous, oral or written promises, or agreements regarding the subject matter hereof (including any statements made on the program webpage) and understand that this is the entire agreement between me and the Released Parties as to the subject matter hereof and cannot be modified or changed in any way except in a writing signed by an authorized. I expressly agree that the foregoing rules, liability waiver, and indemnification provisions are intended to be as broad and inclusive as possible for protection of all said parties including myself.

I agree to abide by all safety-related rules while participating and to attempt only such as I feel capable of performing safely.

I understand the importance of rules and agree to follow all instructions, guidance, and warnings given by Beverly Elizabeth Wheeler, and her duly authorized representatives.

I take full responsibility for my experience while participating in this offering and any and all ceremonies and activities.

I am committed to honoring these terms and approaching the program with the utmost integrity and respect.

I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.

Healing Agreement Contract

Rituals, ceremony, divinations, and work with the Otherworld offer powerful insight, deep connection, unique modes of transformation, and tremendous opportunity for true and long lasting healing.

Working with the Otherworld also means working with the unknown, and the unknown could potentially include risks. When we place ourselves in relation to and in communion with the elements, with nature, with Spirit, we humans are often humbled by the power of these forces in both material and immaterial ways.

Thus, I understand that in partaking in any private sessions, ceremonies, rituals and/or ongoing mentorship or training, I am engaging in activities which contain known and unknown risks. I am engaging in this work in accordance with my own free will and as such I assume responsibility for any physical or non-physical injury, emotional distress, or harm to myself and my property incurred in connection with these rituals, ceremonies, and sessions.

There is no precedent for any such risks or injuries and we do not wish for any injury or mishaps now or in the future. This consent form is not an energetic invitation to any negative vibrations and/or beings seeking any kind of discord that hinders true healing, wholeness, and harmony in any way, shape, or form.

With full understanding, I release Beverly Elizabeth Wheeler, Earth's Amulet, and their representatives from any and all liability.

I am responsible, accountable, empowered and capable of healing.

Community Values Contract

Based on what we have learned in sitting in community spaces, we feel it is important to bring your attention to a community value system and commitment of this nature. This is because this type of ceremonial gathering is both a great learning and a great opportunity of growth for each of us to be open enough to learn new ways of being with one another so as to return to a heart-centered and healing village.

In this, I understand that as part of this offering I enter a temple space where heart-centered relations and respect for Liv Mokai, the container, the protocols, and each being in the circle are essential.

As such, I acknowledge that I am committing to actively and consciously cultivating the following during our journey to the best of my ability:

1. I commit to actively cultivating and showing up with heart-centered energy and care.

2. I commit to actively cultivating and showing up with transparency, vulnerability and respect.

3. I commit to actively cultivating and showing up with love, reverence, and respect for The Sacred- including Liv Mokai and the spirits that support our journey.

4. I commit to actively cultivating and showing up with humbleness, openness, and self-accountability- even when triggered and/or if Liv Wheeler brings something to my attention that is out of my current view/awareness. I commit to tuning into my curiosity and noticing what comes up for me. I commit to asking for support when I may be in need of it.

5. I commit to cultivating an earnest awareness of how each of our energy affects the whole of the community, and in that, creating a space where blame and projections can be replaced by a responsibility for one's personal lived experience and an awareness for how that shades and shapes an experience. I commit to continuously being curious about the subconscious stories and wounds that exist within my being and may try to block me from growing and connecting with both Spirit and all of the community.

In addition to the aforementioned Principles of Community, I am aware of and can whole-hardheartedly commit to the following:

1. I understand that this is a dynamic spiritual process created to ignite, engage, and spiritually train which elicits growth in community, in healing practices, and our lives at large. I commit to being an active and engaged participant for the duration of my membership. I commit to the full arc of this transformation of my own free will and will commit to holding this in my awareness for the duration of my membership.

2. I understand that spiritual teachings and ceremonial classes can feel unpredictable and feel differently each time. I understand that Liv Wheeler's teachings may also not feel linear or easily contained within western constructs of learning. I commit to both respecting this and actively entering into the unknown with trust for the duration of our journey.

3. I understand that in Western consumer culture, there is often an unspoken energy centered around entitlement that can enter the ritual, ceremony, and class spaces. I commit to being mindful of this energy so as to approach the membership, circles, ceremonies, and Liv Wheeler with a respectful heart.

4. I understand that Earth's Amulet strives to create spaces where everyone may feel seen and heard without judgement. And as such, I understand that we are each to try not to give advice and will also try not to take anything communicated personally. When in doubt, I understand that speaking from a place of my own experience and from the heart space is a good place to start.

5. I understand that the sacred space of the calls and this community is confidential and should be treated with the utmost respect. I commit to honoring each of our community members by sharing and communicating with respect and compassion to the best of my ability whilst also honoring their confidentiality.

6. I understand that this is a space in which grief, anger, and authentic expression is most welcomed. Nevertheless, I commit to finding a way to express this such that it is not directed at another community member or facilitator. I understand that if I need support in finding a way to do so, I can ask for support.

7. I sign this agreement of my own free will with the earnest intention to do my best to cultivate and uphold these Community Values with integrity, respect, and heart for the full arc of our journey.
